Medicine Hat Police Service has made certain records available outside of the formal FOIP Access request as permitted by the FOIP Act. The following guide explains the qualifying records and application process.

Case Summary from specified category of minor property occurrence - $50.00 (incl GST) per occurance
- Lost or Stolen Property - Mischief
- Theft from Auto - Break & Enter - no suspect identified
- Stolen / Recovered Auto - Full AB Collision Rpt & Statements (with consent)
Production of qualifying multimedia files - $65.00 (incl GST) per media file
- Audio/Video
- Photos

- Your own witness statement
- The "Driver's Copy" of the AB Collision Rpt. Self Serve your copy via GOA Online Collision
How to Make a Routine Request
To act on behalf of your client/insured, you must provide the following.
- Signed Authorization to Release Information/Consent to the disclosure of individually identifying info from your client/insured. You may use your own agency form or ours
- Must be dated within 90 days of the request
- Confirmation of the file number being requested
- A photocopy of your client's government-issued photo ID
- Complete the MHPS Routine Request form or submit a request letter from your agency that contains the following:
- MHPS file number
- Client's name & date of birth
- Type of incident
- Incident date/time and location address
- Contact name and email address of preferred recipient.
*Please note: Should you fail to retrieve the released records within the allotted time period or if you require MHPS to resend or redirect a release to another representative, your subsequent request may be subject to a $25.00 processing fee
- A cheque or money order payable to "Medicine Hat Police Service" for appropriate fee

Requests are queued for processing once all forms, supporting documentation, and payment is received and deemed to be in good order.
All requests are processed in the order in which they are received.
To request your own information regarding police reports that meet the previously stated criteria, the following two (2) options are available:
Make a Request In Person:
- Attend the Medicine Hat Police Service front counter at 884 2nd St SE in Medicine Hat, AB.
- Complete our Routine Request form
- Present proof of identification in the form of one (1) piece of valid government-issued ID to our front counter staff upon request.
- Examples of accepted photo identification includes, but is not limited to:
- Driver's License or Identification Card
- Passport
- Permanent Residence Card
- Indian/Metis Status Card
- Examples of accepted photo identification includes, but is not limited to:
- Submit payment as indicated on your request form using cash or debit.
*MHPS will not accept payment via credit card*
Submit Request by Mail:
- You may submit a request via mail by submitting an application package that includes the following:
- Completed Routine Request form
- Photocopy of one (1) piece of valid government-issued ID as described above. Photocopy must be clear and legible.
- Payment in the amount indicated on your request form based on report selection. Payment must be in the form of cheque or money order.
- Please do not send cash in the mail

Processing Times
Our goal is to process all Routine Requests within 4 weeks of receipt and approval. This is subject to change without notice and may vary due to the volume of requests received.
Please be aware that, due to the volume of requests received, the Office of the Information Coordinator will not respond to any status update requests during the processing time period.
